Generali Salud Opción Premium — Health Insurance Spain
↓ Jump to Frequently Asked QuestionsGenerali's flagship Spanish-language plan — the most widely recognised by Spanish consulates worldwide and the long-established choice for residency applicants.
Who Is Salud Opción Premium For?
Best fit for clients comfortable with Spanish-language documentation and customer service. The Mi Generali app is available in English but the customer-service phone line is primarily Spanish.
Salud Opción Premium has been issued to NLV applicants for over 25 years. Some Spanish consulates (notably London, Edinburgh, Manchester, Dublin, Houston and Buenos Aires) recognise the plan name immediately and process certificates faster.
Particularly cost-effective for clients aged 55-70 — typically 10-15% cheaper than EXPAT Medcare equivalents at older ages.
Family policies are well-priced and the broad provider network suits clients across all Spanish regions, not just expat-dense coastal areas.
Salud Opción Premium has the broadest provider network of any Generali plan in Spain — about 8,000 additional consultants beyond the EXPAT Medcare network, particularly strong in Madrid, Barcelona and inland regions.
What's Included
Salud Opción Premium is Generali's longest-established premium private health plan in Spain, in continuous offer for over 25 years. It is structured slightly differently from EXPAT Medcare — Spanish-language by default, broader provider network, but with the same core benefits.
- Zero co-payments (sin copagos) — full visa-compliance, no per-service fees, every covered service paid directly by Generali.
- Broadest Generali provider network in Spain — approximately 60,000+ medical professionals, 280+ hospitals and 6,500+ private clinics. Particularly strong in Madrid, Barcelona, Valencia, Bilbao and inland regions where EXPAT Medcare network density is lower.
- All medical specialties — full GP, specialist, surgical and emergency cover with no GP referral required.
- Hospitalisation and surgery — private room as standard, full inpatient cover, scheduled and emergency surgery.
- Diagnostic tests — full radiology, blood, biopsy and endoscopy at network providers.
- Mental health — psychology and psychiatric care at network providers (network larger than EXPAT Medcare for mental-health specifically — most large Spanish cities have multiple Salud-network psychiatrists).
- 24-hour emergency assistance line — primarily in Spanish, with English support available on request.
- Visa-compliant certificate — Spanish-language as standard, English translation available on request. The plan name"Salud Opción Premium" is well-recognised by all Spanish consulates and Extranjería offices.
- Continuity for switchers — waiting periods waived for clients switching from another Spanish private health insurer (Sanitas, Bupa, Adeslas, AXA, DKV) with continuous prior cover.
What's Not Included
Salud Opción Premium is a fully-featured Spanish health plan, but a few key features differ from EXPAT Medcare that may matter to international clients:
- English-language documentation — Salud Opción Premium documentation is primarily Spanish. English summaries and translated certificates are available on request, but the policy schedule, claims forms and customer-service phone line are Spanish-first. EXPAT Medcare is English-first.
- Out-of-network reimbursement — Salud Opción Premium does not include reimbursement at non-network clinics. EXPAT Medcare Prime adds 80% reimbursement; Top adds worldwide reimbursement.
- International cover — Salud Opción Premium is Spain-only outside emergencies. Emergency travel assistance is included up to €30,000 for trips of up to 60 days.
- Age limit — Salud Opción Premium new policies are accepted up to age 70 at policy start (EXPAT Medcare extends to age 74).
- Mi Generali app features — the English-language version of the Mi Generali app supports Salud Opción Premium but with fewer features than for EXPAT Medcare. Telemedicine in English is more limited.
- Pre-existing conditions and dental — same exclusions and add-on options as EXPAT Medcare. Dental is optional, pre-existing conditions are subject to underwriting.

Frequently Asked Questions — Salud Opción Premium
Salud Opción Premium is Generali's Spanish-language equivalent of EXPAT Medcare Standard — same hospital network, same visa compliance, same cover, but with all documentation and customer service in Spanish only. Often a few euros cheaper than EXPAT Medcare. Here are the questions we hear most often.
Almost — same Spanish hospital network, same cover types, same visa compliance, same DGS registration. The two differences: documentation (Salud Opción Premium is Spanish-only versus EXPAT Medcare's English support) and price (Salud Opción Premium is sometimes a few euros per month cheaper). For Spanish-fluent clients, Salud Opción Premium offers virtually all the same benefits at a slightly better price.
Yes — fully visa-compliant. Same DGS-registered Generali España insurer, same no-copayment terms, same full Spain coverage. The certificate is in Spanish (which is what consulates need). For visa-only purposes, Salud Opción Premium may be the more economical choice. The trade-off is that all subsequent policy admin is in Spanish.
Choose Salud Opción Premium if: you're confident in Spanish for medical and admin matters, comfortable with Spanish-only documents, and want the small premium saving. Choose EXPAT Medcare Standard if: you prefer English documents and customer service (most British and Irish clients prefer this), value the English-language app, or anticipate needing to chat with customer service in English. The medical care delivered is identical.
Typically €3-€10 per month cheaper at most ages — around 5-10% saving. The exact difference depends on age and area. Over a year that's €40-€120 saved, which can matter to budget-conscious clients but may be modest relative to the documentation difference. We quote both side-by-side so the choice is transparent.
Yes — at annual renewal you can switch between Generali plans, including between Salud Opción Premium and EXPAT Medcare. Continuity of cover preserves any waiting periods served. Some clients start on Salud Opción Premium for visa purposes (cheapest visa-compliant option), then switch to EXPAT Medcare once they want English customer service.
Identical to EXPAT Medcare Standard — full Generali Spanish private network including HCB Hospital Dénia, HLA Vistahermosa Calpe, Clínica Benidorm, and a wide GP and specialist network across the Costa Blanca. The hospital network is product-tier-based, not language-based; switching language doesn't change clinical access.
Yes — identical waiting periods. Most basic services covered from day 1; hospitalisation and surgery 6 months; childbirth 8 months. Continuity transfers from prior insurers. Visa applicants have waiting periods waived where possible.
Costa Blanca hospitals are well-prepared for English-speaking patients regardless of which Generali plan you have. HCB Dénia, HLA Vistahermosa, Clínica Benidorm and most major Marina Alta clinics have English-speaking reception, English-speaking specialists in many fields, and translation services where needed. The plan documentation is in Spanish; the medical care is multilingual.
Honest answer: rarely, for British or Irish clients. The €40-€120 annual saving is small relative to the convenience of being able to read your own policy, app and certificates without translation. Most expats find the small premium for English documentation pays off in confidence and clarity. Salud Opción Premium is best for Spanish-fluent clients (long-term Spanish residents, Spanish-Latino clients, retirees who learnt Spanish in detail).
Yes — the Generali app is in Spanish for Salud Opción Premium clients. It handles policy admin, network search, claim history, telemedicine bookings and the digital insurance card. EXPAT Medcare Standard's app is the English-language version. The functionality is identical; the language is the difference.
Yes — annual auto-renewal is standard, with premium adjustment based on age band and any general policy updates. We send renewal notice in good time (typically 60 days before renewal) and recommend an annual review of cover. Cancellation requires written notice typically 30 days before renewal under Spanish insurance law (Ley 50/1980).
Yes — a family policy can mix the two plans, though it's unusual. More commonly, the whole family aligns on either Salud Opción Premium or EXPAT Medcare. The administrative simplicity (single renewal, single contact) is preserved either way. We can structure the policy however suits your family best.
