Top is the only EXPAT Medcare tier with 80% reimbursement worldwide — including private treatment in the United States, United Kingdom, Switzerland, Asia and Australasia.
The annual reimbursement cap is substantially higher than Prime — typically €300,000 for medical and €100,000 for surgery. Suitable for clients with cancer histories, complex diabetes, autoimmune conditions, or post-transplant care.
Snowbirds splitting time between Spain and the US, business travellers with regular Asian or African trips, and expats with healthcare needs in multiple countries.
Clients who simply want the best private cover available without compromising on hospital choice, room category or specialist availability.

What's Included
EXPAT Medcare Top is Generali's flagship private medical insurance plan in Spain. It includes everything in Prime, plus four major upgrades that justify the premium for clients who need them.
- 80% worldwide reimbursement — for the first time in the range, this includes the United States, United Kingdom, Canada, Switzerland, Asia, Australasia and Africa. You can use any private clinic, anywhere, and Generali pays 80% subject to the annual maximum.
- Higher annual reimbursement caps — typically €300,000 for medical treatment and €100,000 for surgery, compared with €15,000-€30,000 on Prime. Sufficient for chronic-condition treatment and complex surgery.
- Unlimited psychology sessions — no annual cap, plus access to specialist therapy programmes (eating disorders, addiction recovery, trauma therapy) and full psychiatric care.
- Premium room category — single-bed private room with companion bed and en-suite bathroom guaranteed at all network hospitals. (Standard and Prime offer single rooms but not always the premium category.)
- Concierge medical service — Generali Salud premium concierge can book appointments, arrange second opinions from international specialists, organise medical evacuations and provide bilingual case management.
- Maternity included as standard — full antenatal, childbirth and post-natal cover, after the 8-month waiting period from policy start.
- Worldwide travel assistance — up to €100,000 emergency medical cover for trips of up to 180 days outside Spain (Standard and Prime are limited to €35,000 and 90 days).
- All Prime tier benefits — full Spain network, optional dental, optional hospital cash benefit, telemedicine, and visa-compliant certificate.
What's Not Included
Top tier is the most comprehensive available in Spain but a few exclusions still apply:
- Pre-existing conditions disclosed at application — same medical underwriting principle applies. Conditions may be excluded or loaded (for severe cases). Always declare honestly.
- Cosmetic procedures — not covered unless medically necessary (reconstructive surgery after accident or illness is covered).
- Long-term care or nursing-home stays — Spanish health plans do not cover care-home or residencia stays.
- Experimental or unproven treatments — not covered unless approved by Spanish or EU medical authorities.
- Treatment in countries with US sanctions — Iran, North Korea, Cuba, Syria — not covered.
- Reckless or self-inflicted injury — standard exclusion across all tiers.

Frequently Asked Questions — EXPAT Medcare Top
EXPAT Medcare Top is the premium tier of Generali's English-language range. Designed for affluent, mobile or cross-border clients who want the highest cover limits and worldwide treatment reimbursement.
Top adds three premium features over Prime: worldwide medical reimbursement (Prime is Spain-only with limited emergency travel); the highest annual cover limits in the Generali range; and enhanced wellness/preventive cover. Top is positioned as a global healthcare plan with Spain as the primary base, rather than a Spain plan with travel extension.
Top reimburses 80% of medical costs incurred anywhere in the world up to the annual cover limit. You pay the foreign provider, submit the bill and itinerary, and Generali reimburses 80% in euros to your Spanish bank account. Excludes the United States by default (US can be added by extension). Significantly more expensive than Spanish-only treatment because international rates are higher; Top funds it appropriately.
Three client profiles: (1) UK-Spain split residents who spend significant time in both countries and want full UK private treatment access; (2) international professionals on Spanish residence whose work takes them abroad regularly; (3) high-net-worth retirees with global travel patterns. Top is overkill for clients who plan to stay primarily in Spain.
Standard Top includes worldwide reimbursement EXCLUDING the USA, Canada and the Caribbean — these high-cost jurisdictions need a specific extension. The US extension typically adds 30-50% to the premium because of significantly higher US medical costs. Required if you visit the US regularly or have US property. Confirm at quotation.
Everything in Prime, plus the most exclusive private hospitals: top-tier private rooms in Madrid (Hospital Universitario Sanitas La Moraleja, Hospital Universitario Quirónsalud Madrid, Cemtro), Barcelona (Hospital Universitari Dexeus, Centro Médico Teknon), and the premium Costa Blanca hospitals. For ultra-specialist treatment (oncology, cardiology, neurosurgery), Top gives access to the best Spanish medical centres.
Same underwriting as Standard and Prime — pre-existing conditions are assessed individually with possible loadings or exclusions. Top doesn't accept conditions that other tiers would decline. The premium tier reflects broader cover and higher limits, not different acceptance criteria.
Probably not. For one or two trips a year of 1-2 weeks each, Standard or Prime travel cover (€15,000-€50,000 emergency medical) is normally sufficient. Top is justified when you spend 30%+ of the year abroad, have routine treatment in another country (an established UK GP or specialist relationship), or have a specific medical need likely to be treated outside Spain.
You pay the foreign provider in their local currency, then submit: original receipts, doctor's report, procedure description, and your bank details. Generali reimburses 80% in euros to your Spanish bank account, typically within 30-60 days. The foreign provider doesn't bill Generali directly — it's a pay-and-claim model. Keep all documentation; without it, claims fail.
Yes — Top includes annual wellness benefits: a comprehensive health screening (chequeo médico) once per year at no charge, optional preventive consultations, mental health/psychology sessions (typically up to 25 per year), nutrition consultations, and optional executive medicals. These extras significantly increase the value of the plan for clients who use them.
Yes — downgrades are allowed at annual renewal. Downgrading preserves continuity (no new waiting periods), and you can upgrade back later if circumstances change. Some clients hold Top during years with significant travel/treatment needs, then drop back to Prime in quieter periods. We advise on this on a case-by-case basis.
New applicants are accepted on Top up to age 74 at policy start (same as Standard and Prime). Once on cover, the policy renews for life. Older applicants face higher premiums but the cover continues. For applicants over 74, alternative international health products are available — often more appropriate at that age than starting on a Spanish-domestic plan.
Cigna Global and Bupa Global are designed for highly mobile international expats, with global hospital networks and limits sometimes higher than Top. Their downside: substantially higher premiums (often 50-100% more than Top), and Spanish documentation can be patchy for visa purposes. Top hits a sweet spot for Spain-resident expats who want global treatment access without paying full international rates.
